Course Description:

For patients living with diabetes, poor glucose control increases the risk of developing serious complications and hospital admissions. Improvements in treatment adherence, persistence, and intensification of therapy require proactive collaboration between the healthcare team and the patient. Diabetes care is rapidly becoming more technology-focused, with smart devices, mobile applications, and continuous glucose monitors (CGM), which provide more accurate data. Note: This series is supported by an Abbott Diabetes Care educational grant.

Course Objectives:

  • This collection of activities demonstrates how clinicians and patients can use continuous glucose monitoring (CGM) technology and glucose data from ambulatory glucose profile (AGP) reports to identify glucose patterns and individualize their approach to care.
  • Patients can learn how physical activity, nutrition, and medication impact their glucose levels and identify episodes of hypoglycemia.
